When I schedule your first therapy session, your email address will be used to preregister you in the Client Portal. You will receive an email to set up your portal and complete all intake paperwork electronically. These include:
- Client Information
- Payment Authorization Form
- Disclosure Statement
- Notice of Privacy Practices
- Fee Agreement
- Release Information
$200 / 50-minute session (includes shorter EMDR sessions)
